CUNY students who will be graduating at the end of the fall 2025 term may request to have their previously designated preferred first name (and preferred middle name, if applicable) listed on their diplomas.
Submission of this form is voluntary. No action is required if you would like your diploma to reflect your legal name of record. Please complete and submit the form below by January 31, 2026. Late submissions will not be honored.
